I’d the same problem, it only popped on adventure. :P, I'm having the same problem on ps4 but with the weapon mods, I have 16 but you need 15 for trophy and still have not got it, Edit: I just got another mod from a boss I just found that puts me at 17 mods now and just unlocked the trophy. Now the way I am showing is the cheese way. The game received generally positive reviews upon release and more than 1 million units have been sold.
A really good mod I would recommend is the explosive shot since it deals a lot of damage. Content is generally suitable for ages 17 and up.

Getting that trophy solo was super easy though, shouldn't take more than 2-3 attempts if you do it when you're done with everything else. Which boss have you been attempting? Easily done vs Singe once fully upgraded. I got the achievement doing the Ent boss where I got hit by several adds but not the boss itself. You can die and come back. This video helped a lot.
